Outcomes We Measure
We continually monitor the impact of our training through measurable outcomes across five key areas.
In Safety, our goal is to reduce self-harm incidents, aiming for a 40% reduction within 12 weeks.
Health focuses on medication concordance, with a target of 100% compliance supported by weekly reviews.
Under Engagement, we measure progress by encouraging sustained community participation, aiming for each person to take part in at least two regular activities by week eight.
Stability is evaluated through a reduction in hospital readmissions, with an expected decrease of 50% or more within 12 weeks.
Finally, Voice and Control reflects our commitment to person-led support, with weekly records and feedback sessions ensuring each individual’s choices shape their own care. These outcomes demonstrate how continuous staff learning translates into stronger, more positive results for the people we support.
